Bartley Healthcare offers comprehensive services to patients recovering from illness, surgery or injury who have been discharged from an acute care setting but are not ready to return home. With separate entrance, living and treatment areas, our post acute services offer medically complex care, as well as short-term in-patient rehabilitation 7 days a week.
Subacute services are provided using clinical pathways. These diagnosis-specific protocols enable us to deliver a cost-effective, positive outcome treatment plan for patients with functional decline or specific medical conditions. Our interdisciplinary team guides the patient from admission to discharge and beyond. The plan is carefully followed to ensure continuity of care and positive results for a successful return home.
Using a team approach, our outcome-oriented rehabilitation program empowers patients to rebuild lost or diminished abilities and regain independence.
Our program includes intensive therapy provided by licensed physical, occupational and speech therapist under the supervision of a board certified physiatrist.
Individualized care plans with assessment and evaluation are developed for each patient.
Bartley Rehabilitation Services integrates the services of Accelerated Care Plus (ACP) into our therapy program. ACP provides us with traditional rehabilitation therapy devices (exercise equipment, cycles, etc.) plus the advantage of evidence-based clinical programs. This superior combination not only enhances patient care, quality of life and independence, it helps promote superior outcomes and helps patients optimize functional gains.
