Subacute Care

Bartley Healthcare, a recognized leader in subacute care, offers comprehensive services to patients recovering from an illness, surgery or accident who have been discharged from an acute hospital setting but are unable to return home. With a separate entrance and separate living and treatment areas, our subacute services offer medically complex care as well as short-term in-patient rehabilitation.

Medically Complex Care:

Subacute services are provided using clinical pathways. These diagnosis specific protocols enable us to provide a cost-effective, positive outcome treatment plan for patients with functional decline or specific medical conditions. Our interdisciplinary team guides the patient from admission to discharge, and beyond. The plan is carefully followed to ensure continuity of care and positive results for a successful return home.

In-Patient Rehabilitation:

Bartley Healthcare Nursing and Rehabilitation offers a comprehensive in-patient, short term rehabilitation program, available 7 days a week.

Using a team approach, our outcome-oriented rehabilitation empowers patients to rebuild lost or diminished abilities and regain independence. We focus on restoring optimal functioning and returning the patient to the community to live with confidence and independence. Social workers are available to meet the needs of our subacute patients at all times, and a discharge plan is set on the day of admission to produce a timely and successful return home.

Our program includes intensive therapy provided by licensed physical, occupational and speech therapists under the supervision of our board certified physiatrist. Individualized care plans with assessment and evaluation are developed for each patient. Special rehabilitation programs include:

Fix-It Program—provided to all patients who utilize wheelchairs and/or walkers to ensure proper functioning and appearance of all adaptive equipment.

Brace Clinic—held bi-weekly by a team consisting of a physiatrist, physical therapist, rehab technician and an equipment supplier to address all patient orthodic/prosthetic needs to make certain the device is properly fitted and utilized.

Wheelchair Clinic—held bi-weekly by our rehab team consisting of physical and occupational therapists, a rehab technician and an equipment supplier to identify patients with positioning problems and determine wheelchair and adaptive equipment needs.

Rehab Dining Room—patients experiencing swallowing difficulties are seen by speech therapists during mealtime in order to ensure safety in swallowing and to maximize their highest potential for functional independence.

Bartley Healthcare's innovative and creative Therapeutic Recreation program utilizes various interventions to treat physical, social, cognitive and emotional conditions associated with illness. We assist patients in maintaining optimum levels of independence, productivity and well-being so they may re-enter the mainstream of community life. All programs are led by a nationally certified therapeutic recreation specialist and physical and occupational therapy assistants and include:

STRIVE [Skilled Therapeutic Recreation for Independence & Vital Empowerment)— an ambulatory leisure group which assists in increasing functional capabilities and independence.

LAFS [Leisure Activities and Functional Skills]— a seated leisure group emphasizing socialization and self-expression.

Community Resource Education—provides organized community-based recreation programs.
 

bullet

Leisure Education—helps patients become more active participants in their health care by using activity to cope with the stress of illness.
 

bullet

Cognitive Re-training—in conjunction with speech therapy, this program provides training in skills such as sequencing, problem solving, decision making and communications.
